What is buttock fat transfer?
Buttock fat transfer is a surgical procedure that increases the volume and improves the shape of the buttocks using the patient's own fat, previously removed by liposuction from areas with fat deposits.
This treatment adds projection to the buttocks and improves the overall silhouette, with a result in keeping with each person's anatomy and without implants.

What does a buttock augmentation at IdB involve?
Buttock augmentation by fat transfer (known as BBL) begins with liposuction in areas with excess fat, such as the abdomen, flanks or back, using thin cannulas to remove the fat cells. Once harvested, the fat is centrifuged and purified to separate the living fat cells from blood and oil, ensuring that only high-quality tissue is reused. Finally, the surgeon makes small incisions in the buttocks to reinject the purified fat into the subcutaneous plane, without infiltrating it into the muscle, shaping the desired volume and projection in a natural, permanent way.
Personalized medical assessment
Every patient has a different anatomy, tissue quality and aesthetic goals. Before considering any treatment, an individualized medical consultation is essential. To book the appointment, a payment of €80 is required in advance. If you cannot travel, the first visit can also be done by video call with the surgeon who would operate on you: it costs €50, which is deducted if you decide to have surgery.

In the days before surgery, every detail of the procedure is planned, defining the areas from which the fat will be removed and how it will be redistributed to achieve a harmonious, natural result.

Fat is removed by liposuction from areas such as the abdomen, flanks or thighs, processed and then injected into the buttocks to increase their volume and improve their shape. The fat is distributed precisely in the subcutaneous plane to achieve an even result.

Who is not a candidate: contraindications for buttock fat transfer
Although buttock fat transfer is a highly sought-after technique because it offers augmentation with your own fat and natural results, not everyone is a suitable candidate for this procedure. As with any surgery, safety must always come before the aesthetic result.

We take into account the desire to improve body contour, general health, tissue quality, the donor fat available and expectations about the result. Being a good candidate does not depend only on wanting surgery, but on being able to have it with the proper medical safeguards.
Contraindications for buttock fat transfer

Risks and possible side effects of buttock fat transfer
One of the reasons buttock fat transfer has become one of the most in-demand body surgeries is the possibility of improving contour using the patient's own fat, achieving a natural, harmonious result. However, like any surgical procedure, it must be performed under strict medical criteria and by surgeons with specific experience in this technique.

Harvesting, processing and injecting the fat require technical precision and individualized planning to ensure both the viability of the graft and the patient's safety.
If after surgery you experience severe, disproportionate pain, a fever above 38 °C, difficulty breathing, chest pain, a sudden increase in volume in one buttock, discharge from the incisions, spreading redness or shortness of breath, it is important to contact the clinic immediately or go to the nearest emergency department. Early detection makes it possible to act quickly in the event of any significant complication.
Expected, mild reactions (most resolve on their own within 7–14 days)
These symptoms are part of the body's normal response after surgery and usually improve gradually with the post-op care prescribed:
- Moderate pain and a feeling of muscle soreness in the buttocks and liposuction areas during the first few days.
- Swelling and edema in both the donor areas and the buttocks during the first few weeks.
- Mild or moderate bruising that gradually disappears within 2–3 weeks.
- A feeling of tightness or temporary hardening in the treated areas.
- Temporary reduction in skin sensation in the liposuction or injection areas.
- Initial asymmetries related to swelling and the transferred fat settling in.
- Slight partial reabsorption of the injected fat, considered normal during the first few months.
- Tiredness or mild fatigue during the first week after surgery.
All these reactions are usually part of the normal recovery process. Most patients can resume sedentary activities from 10 to 15 days, avoiding sitting directly on the buttocks during the first few weeks and reintroducing strenuous physical exercise from 6–8 weeks, depending on medical progress.
Less common effects
Although most patients progress well and without significant incidents, there are some less common side effects that may appear during the recovery process:
- Uneven partial reabsorption of the fat, with small differences in volume between the two buttocks.
- Swelling that persists for several weeks, especially in patients prone to edema.
- A feeling of hardness or small temporary lumps due to the fat integration process.
- Prolonged hypersensitivity or numbness in some treated areas.
- Discomfort when sitting during the first few weeks, especially if the post-op recommendations are not followed.
- Temporary changes in skin texture or mild fibrosis in the liposuction areas.
- The need for a later touch-up to optimize symmetry or final volume in certain cases.
Uncommon complications (preventable with rigorous medical practice)
- Postoperative infection in the liposuction or fat injection areas.
- Seroma (fluid buildup) after surgery.
- Significant hematomas that may require medical monitoring or drainage.
- Excessive reabsorption of the transferred fat, leading to partial loss of the volume achieved.
- Contour irregularities or internal fibrosis resulting from the healing process.
- Persistent asymmetry between the two buttocks.
- Fat necrosis with the formation of small hardened lumps.
- Prolonged changes in sensation in the treated areas.
