What is nasal feminization?
It is a specialized surgical procedure that aims to bring the structure of the nose into harmony with softer facial features. It is not simply about "making the nose smaller," but about reducing projection, narrowing the bridge and subtly lifting the tip to achieve a more delicate profile that draws attention to the eyes and lips.
Types of nasal feminization
Conventional rhinoplasty
It uses manual tools such as the chisel and mallet to fracture and reshape the bone, which tends to cause more trauma to the soft tissues and lead to bruising or "black eyes" during the first few weeks.
Ultrasonic feminizing rhinoplasty
uses a high-tech device (piezotome) that relies on vibrations to file or cut the bone with millimeter precision, fully sparing the blood vessels and the surrounding skin.
What does the nasal feminization process involve?
The procedure consists of reshaping the bone and cartilage structures through ultrasonic rhinoplasty under general anesthesia. It is performed using an open technique that makes it possible to sculpt the dorsum and lift the tip with millimeter accuracy, achieving a softer, more feminine profile without affecting breathing function.
Assessment and understanding
In this initial phase, the surgeon analyzes the patient's nasal anatomy and facial harmony. This is the time to align expectations
A €80 payment is required in advance to book the appointment. If you cannot travel, the first visit can also be done by video call with the surgeon who would operate on you: it costs €50, which is deducted if you decide to have surgery.
Preparing for the surgery
Before surgery, full preoperative tests are carried out (blood work, electrocardiogram) and specific instructions are given, such as fasting and stopping certain medications or smoking. This stage ensures that the patient arrives in the operating room in optimal health.
The surgery
The surgery takes between 2 and 4 hours. The piezotome (ultrasound) is used to file the bone cleanly and safely. At the end, an external thermoplastic splint is placed to protect the new shape, and at IdB no nasal packing is used in approximately 95% of procedures, so patients usually leave the operating room breathing through their nose.

What makes a nose look more feminine?
There is no such thing as a feminine nose and a masculine nose, only features that are perceived as softer or more pronounced. Nasal feminization works on those specific features, and knowing which they are helps explain what can and cannot be changed.
The dorsum
A straight line, or one with a slight concavity, is perceived as softer than a dorsum with a hump. This is what changes the profile the most, and it is addressed by filing the bone with ultrasound.
The tip and the nasolabial angle
A slightly more lifted and defined tip, with a more open angle between the nose and the lip, softens the overall look. It is the most delicate area and the last to settle after surgery.
The base and the nostrils
Reducing the width of the base narrows the nose when seen from the front. This has to be assessed carefully: narrowing it too much compromises breathing.
The radix and its relationship with the forehead
The radix is where the nose begins, between the eyebrows. Its position changes if the brow ridge is reduced, so when the forehead is part of the plan it makes sense to think about the two areas together rather than separately.
The distance between the nose and the lip
A short upper lip is perceived as softer. That is not resolved with the nose but with a lip lift, and it is common to plan the two together when the proportions call for it.
And one clarification
Not every trans woman wants or needs this surgery. The consultation is also for that: deciding whether it is necessary and which part of the plan makes sense in your case.

What are the risks of nasal feminization?
What you should know beforehand
Nasal feminization is a rhinoplasty, and rhinoplasty has one of the highest revision rates in cosmetic surgery. That is not a sign that something has gone wrong: the nose heals over months, and a proportion of cases need a later adjustment. Saying so beforehand avoids having to explain it afterwards.
The procedure is performed using an open technique when the plan requires it, which leaves a scar on the columella, at the base of the nose, that becomes barely noticeable over time.
Expected effects
- Swelling and bruising around the eyelids during the first two weeks.
- An external splint for the first few days and nasal congestion, although in most cases no packing is used.
- Altered sensation in the tip, which recovers gradually.
- A result that keeps refining for months: the tip is the last area to take shape.
Less common risks
- Breathing difficulty due to narrowing of the airway when the dorsum or the base is reduced.
- Irregularities of the dorsum that can be felt or seen as the swelling goes down.
- Asymmetry of the tip or the nostrils.
- The need for a secondary rhinoplasty as an adjustment.
- A visible columella scar, with the open technique.
- Infection or bleeding that requires treatment.
Who is NOT a candidate: contraindications for nasal feminization

Frequently asked questions about nasal feminization
Is rhinoplasty recovery painful without nasal packing?
Contrary to popular belief, rhinoplasty is not a painful surgery so much as an uncomfortable one because of the congestion. By not using nasal packing in approximately 95% of procedures, IdB avoids the most unpleasant and painful step of recovery: removing the packing. The patient has a sensation similar to a common cold, but can breathe through the nose from the first day, which reduces that stuffy, overwhelmed feeling and improves rest.
When does full recovery after nasal feminization come?
Although the change in profile is evident from the moment we remove the splint at 7 days, full recovery requires patience. Most of the swelling disappears in the first month, but the nasal tip, which is the area worked on the most in a feminization, can take between 6 and 12 months to show its final, refined shape.
What are the risks of feminizing rhinoplasty?
As with any surgical procedure, there are general risks such as infection, bleeding or a reaction to anesthesia, although at Instituto de Benito these are minimized with the strictest safety protocols. Specific risks may include minor asymmetries, irregularities in the dorsum or slow healing at the tip. However, by using the ultrasonic technique, the risk of tissue damage is drastically lower and the precision of the final result is much greater than with the traditional technique.
Can rhinoplasty be combined with other feminization procedures?
Yes, in fact it is very common to perform it together with forehead feminization or cheek augmentation. Combining procedures allows for complete facial harmonization under a single anesthesia and one recovery period, achieving a smoother, more natural transition of features.
