What is gynecomastia?
Gynecomastia is the enlargement of breast tissue in men. Gynecomastia surgery, also known as male breast reduction, is a surgical procedure designed to remove excess glandular tissue and/or fat from the male chest, giving the chest back a firm, flat and well-proportioned appearance, with a natural, harmonious result.

What types of gynecomastia are there and how are they treated?
Fatty gynecomastia (liposuction)
Indicated when male breast enlargement is mainly due to excess fat. It is corrected with liposuction, achieving a flatter, more defined chest with minimal scarring.
Glandular gynecomastia (gland removal)
Recommended when there is excess glandular breast tissue. It requires direct surgical removal of the gland through a small incision around the areola.
Mixed gynecomastia (combined)
This is the most common form. It combines excess fat and excess gland, so treatment includes liposuction and gland removal to achieve a flat, firm and natural result.
What does gynecomastia surgery involve at IdB?
The procedure is performed after a personalized consultation and may include liposuction to remove localized fat and/or removal of glandular tissue through small, discreet incisions, usually around the areola. The technique is adapted to each case to achieve a natural, balanced result in keeping with the patient's anatomy.

Personalized medical assessment
Your consultation will always be with the surgeon who will perform your procedure. In this appointment we analyze the type of gynecomastia, skin quality, the amount of gland and fat, and the best technique for your case. A payment of €80 is required in advance to book the appointment. If you cannot travel, the first visit can also be done by video call with the surgeon who would operate on you: it costs €50, which is deducted if you decide to have surgery.

Gynecomastia surgery at Instituto de Benito starts from €6,500, varying according to the grade and type of gynecomastia, the technique used, the length of the operation and the tests and follow-up included, and is tailored to each patient.

Who is not a suitable candidate: contraindications for gynecomastia surgery
Although gynecomastia surgery is a safe and common procedure in plastic surgery, there are situations in which it is not recommended or should be temporarily postponed. At Instituto de Benito we always carry out a full medical assessment before recommending any procedure, putting the patient's health and safety ahead of the aesthetic component.
Not every man with increased breast volume is an immediate candidate for surgery. It is essential to identify the cause of the gynecomastia, assess the patient's general health and confirm that he has realistic expectations about the result.
Contraindications and reasons

Risks and possible side effects of gynecomastia surgery
Gynecomastia surgery has a well-established safety profile when performed by a specialized surgeon, in an accredited surgical setting and after a proper patient assessment.
At Instituto de Benito, surgery is performed in an approved operating room, with anesthesia administered and supervised by a qualified anesthesiologist. Follow-up afterwards makes it possible to detect and treat any issue early.
If after surgery you experience severe pain that does not improve with the prescribed medication, a fever above 38 °C, marked local warmth, progressive redness, discharge from the wound, a sudden increase in volume on one side of the chest, difficulty breathing or chest pain, you should contact the clinic immediately or go to the nearest emergency department.
Expected, mild reactions (usually temporary)
These are part of the body's normal response after surgery and resolve gradually over the first few days or weeks:
- Moderate pain or a feeling of tightness in the chest during the first few days, manageable with the prescribed pain relief.
- Swelling and edema in the treated area.
- Bruising around the incisions, which fades gradually.
- Temporary changes in nipple sensation, either increased or decreased.
- Scars that are initially pink or firm, which take months to mature fully.
- Temporary asymmetry while the tissues adapt and the swelling goes down.
Your surgeon will tell you at the consultation when you can resume sedentary activity, work and exercise, because it depends on the technique used and on your progress.
Less common effects
These can appear during recovery or in the first few months. They almost always resolve with clinical follow-up, and only a minority require revision surgery:
- Seroma: a build-up of fluid in the operated area, which may need monitoring or drainage.
- Significant hematoma requiring surgical evacuation.
- Contour irregularities or small steps between the treated area and the adjacent one.
- A depression or hollow under the areola if the gland resection is excessive.
- Residual excess skin when the breast was large and the skin has little elasticity.
- Residual glandular tissue that leaves a noticeable bulge.
- Hypertrophic or keloid scarring in predisposed patients.
- Delayed healing, more likely in smokers and in people with diabetes.
Uncommon complications
These are uncommon, but they have been documented and you should be aware of them before having surgery:
- Surgical wound infection, which may require antibiotic treatment or surgical cleaning.
- Partial necrosis of the nipple-areola complex, more likely in smokers and in extensive resections.
- Permanent loss of nipple sensation.
- Deep vein thrombosis and pulmonary embolism, risks present in any surgery under general anesthesia.
- Anesthetic complications, assessed beforehand by the anesthesia team.
- Recurrence of gynecomastia if the cause that originated it persists.
An individualized surgical indication and proper follow-up are what reduce these risks.

What results can I expect?
A flatter, firmer and better-proportioned chest with a natural masculine contour. Full recovery comes between 3 and 6 months, once the tissues have fully adapted.
Is gynecomastia surgery painful?
Discomfort is usually mild to moderate and is effectively controlled with medication and advanced recovery protocols. Most patients return to their usual routine in 4 to 7 days.
Can gynecomastia come back after surgery?
Under normal conditions, the glandular tissue that has been removed does not grow back. However, significant hormonal changes or significant weight gain could affect the result over time.
Will there be visible scars?
The incisions are small and strategically placed, usually around the areola or in areas that are not very visible, so over time they tend to be practically imperceptible.
When can I exercise or go in the sun?
Exercise: gradually from 4–6 weeks. Sun or beach: after 8 weeks, always with high sun protection on the scars.
Can gynecomastia surgery be combined with other procedures?
Yes. In some cases it can be combined with liposuction of other areas or other body procedures, always after a medical assessment.
What happens if I gain or lose weight afterward?
Moderate weight changes do not usually alter the result significantly. Keeping a stable weight helps preserve the result in the long term.
How long does full recovery take?
Most patients return to normal life in 4 to 7 days, although full recovery comes at around 3 months.
