Breast implant replacement

These are part of the body's normal response after surgery and usually resolve gradually over the first few days or weeks:

• Moderate pain or a feeling of pressure or tightness in the chest during the first 48–72 hours, manageable with pain relief.
• Breast swelling and edema during the first 2–3 weeks.
• Mild bruising around the incisions, which gradually disappears within 1–2 weeks.
• A feeling of muscle tension, especially if the implant plane is changed.
• Temporary changes in breast or nipple sensation (hypersensitivity or reduced sensation), which usually improve over the following months.
• Scars that are initially pink or firm, which can take 12 to 18 months to mature fully.
• Temporary asymmetry while the tissues adapt to the new implant and the swelling goes down.

Most patients can return to sedentary activity after about a week, go back to office work within 7 to 10 days and resume intense physical exercise from 4 to 6 weeks, depending on their progress.

There are some issues that can appear during recovery or in the first few months and that, although generally not serious, are worth knowing about:

• Seroma: a build-up of fluid around the implant or capsule, which may require ultrasound monitoring or drainage.
• Significant hematoma requiring surgical evacuation.
• Persistence or recurrence of a previous capsular contracture.
• Hypertrophic or keloid scarring in predisposed patients.
• Residual breast asymmetry or contour irregularities that may require minor touch-ups.
• Rippling, or implant folds that can be felt or seen, more common in patients with little breast tissue.
• Implant malposition (displacement, descent or rotation), especially in complex secondary surgeries.
• Gradual sagging of the breast due to skin quality or loss of tissue elasticity.
• Delayed healing, especially in smokers, patients with diabetes or patients with multiple previous surgeries.

Most of these situations can be managed with close clinical follow-up, and only a minority require further surgery.

Although uncommon, there are complications described in the medical literature that every patient should be aware of before implant replacement surgery:

• Capsular contracture: hardening of the fibrous capsule surrounding the implant, with possible aesthetic changes or discomfort.
• Periprosthetic infection: in severe cases it may require temporary removal of the implant.
• Rupture or wear of the previous implant detected during surgery or at later check-ups.
• Technical difficulty arising from fibrosis, calcifications or tissue changes caused by previous surgeries.
• Deep vein thrombosis and pulmonary embolism, risks present in any surgery under general anesthesia.
• Anesthetic complications, assessed beforehand by the anesthesia team.
• Necrosis of the skin or of the nipple-areola complex, more common in smokers or in surgeries with significant tissue tension.
• BIA-ALCL (breast implant-associated anaplastic large cell lymphoma): an extremely rare complication historically linked to certain textured implants.
• Future need for further replacements: breast implants are not considered permanent devices and may need to be replaced over the years.

A proper assessment, an individualized surgical indication and correct medical follow-up are essential to minimize risks and optimize long-term safety.
