What is the barcode (perioral lines)?
The "barcode" are the small vertical lines that appear above the upper lip with age or due to habits like smoking. It is treated by combining a medical neuromodulator at low doses and, when necessary, microinjections of low-density hyaluronic acid to fill the lines and rehydrate the area.
Medical Evidence and Support
Perioral lines are a consequence of the aging of the orbicular muscle and the thinning of the upper lip, aggravated by smoking and sun exposure. The Spanish Academy of Dermatology and Venereology (AEDV) describes the condition as one of the most characteristic signs of facial aging. The combined medical-aesthetic approach (neuromodulator + low-density hyaluronic acid ± peeling) is validated by the Spanish Society of Aesthetic Medicine (SEME) as a first-line non-surgical treatment.
What does the treatment of barcode (perioral lines) consist of?
The treatment is carried out entirely in the medical office of the Instituto de Benito in Barcelona or Madrid, in an outpatient session always designed and supervised by a licensed aesthetic doctor. It consists of the following steps:
Personalized Medical Assessment:

Before the treatment, an assessment is carried out with the aesthetic doctor of the Instituto de Benito. They study your case, listen to your objectives, rule out contraindications, and design a personalized plan. The first aesthetic medical visit costs €50 (deductible from the first treatment).
Treatment Preparation

The preparation is simple: the area must be clean and free of creams. The doctor will advise if it is advisable to avoid any medication (anticoagulants, NSAIDs) in the days prior to minimize bruising, and discuss any relevant circumstances for your safety.
Application of Treatment in Consultation

The doctor disinfects the area, applies topical anesthesia if necessary, and performs the microinjections with very fine needles, following the designed plan. The session is carried out entirely in consultation and the patient resumes their routine immediately.
Start seeing yourself the way you want to feel

The results appear within the expected timeframe for the treatment, with a natural and harmonious finish. The doctor provides you with home care instructions and schedules follow-up appointments.

Risks and possible side effects of upper lip wrinkle treatment
Barcode (perioral lines) is a safe treatment when performed by a licensed doctor in an authorized center and with approved medical products. Even so, every medical procedure has a profile of possible effects that it is honest to share with you.
Expected and mild reactions (self-limited in 24-72 h)
- Papules or small wheals at the injection points, which subside in minutes or hours
- Localized redness (erythema) of the area
- Mild bruising (more common in thin skin or if taking anticoagulants/NSAIDs)
- Sensitivity or slight local discomfort
- Mild swelling for 24-72 h
Less frequent effects:
- Mild asymmetries correctable in the touch-up appointment
- Hypersensitivity or itching that subsides in a few days
- Transient post-inflammatory hyperpigmentation if the area is exposed to the sun
- In specific cases: nodules or product irregularities that are corrected with massage or dissolution
Uncommon complications:
- Local or systemic allergic reactions
- Late granulomas due to reaction to the product
- In vascular areas: accidental vascular occlusion (extremely rare, manageable if detected in time)
- In cases of malpractice or professional intrusion: infections, persistent nodules, lipodystrophy or scarring
Our medical group aesthetic medicine
An aesthetic doctor from the IdB team decides whether to work on the barcode only with a neuromodulator, only with low-density hyaluronic acid, or with a combination of both depending on the degree of the mark.

Commitment to facial aesthetic medicine
The vertical lines of the upper lip are one of the most technically demanding areas; at IdB we combine the anti-wrinkle neuromodulator at low doses with superficial hyaluronic acid precisely.
At the Instituto de Benito, patient safety comes before any aesthetic goal. We work with audited clinical protocols, medical CE-marked health products and batch traceability, single-use instruments, dedicated aesthetic medicine rooms, and personalized follow-up between sessions. We never delegate a medical-aesthetic procedure to non-medical personnel.

Frequently Asked Questions
Why do barcode wrinkles appear?
Due to the combination of skin aging, muscle movement of the orbicularis, and factors such as smoking or sun exposure.
Which treatment is most effective?
The combination of a medical neuromodulator at low doses + low-density hyaluronic acid. In advanced cases, it is complemented with chemical peeling.
How much does it cost?
Hyaluronic acid 350 €. Complementary peeling 150 €. Locations in Barcelona and Madrid.
Which treatment is most effective?
The combination of a medical neuromodulator at low doses + low-density hyaluronic acid is the standard. In advanced cases, it is complemented with chemical peeling.
When do I see results?
Visible from the first week, optimal at 2 weeks. Duration: 8-12 months depending on product and technique.
What treatment is most effective for barcode lines?
The combination of a medical neuromodulator at low doses + superficial hyaluronic acid is the standard. In advanced cases, it is complemented with chemical peeling or fractional laser. Monotherapy is less effective: the muscular cause and volume loss must be addressed simultaneously.
Does smoking have an influence?
A lot. Smoking accelerates the appearance and deepening of barcode lines due to the combination of repeated movement when smoking and the effects of tobacco on collagen. If you smoke, the doctor will inform you, and the result will be more limited and shorter-lasting than in non-smoking patients.
Can barcode lines be prevented?
Partially. Preventive measures include: not smoking, daily facial photoprotection, constant hydration of the perioral area with active ingredients (hyaluronic acid, retinoids), and periodic mesotherapy or facial bio-revitalization from the age of 30-35.
When to apply chemical peeling?
When there is very marked texture or if the patient wants to intervene more deeply: specific chemical peel (€150) renews the superficial layer and improves the quality of the skin of the upper lip, complementing the effect of the filler and the neuromodulator.
How soon can the result be seen?
Visible from the first week, optimal at 2 weeks. Duration: 8-12 months depending on product and technique. The patient notices improvement from the mirror the first weekend, before the result is fully settled.
