What is mentoplasty?
Mentoplasty is chin surgery indicated to improve the projection, shape or proportion of the lower third of the face. At Instituto de Benito it is planned on a personalized basis, assessing the facial profile, the jaw, the neck and the patient's expectations in order to choose between augmentation with an implant, reduction or bone reshaping. The goal is a harmonious, natural result that is consistent with your features, with complete medical follow-up before and after the procedure.
To augment the chin, the simplest option is usually to place a silicone implant through a small incision inside the mouth, under local anesthesia. The stitches stay inside and fall out on their own; patients are usually swollen for a few days, but can return to normal life in 7-10 days. Another option, performed by maxillofacial surgeons, involves moving the chin bone itself (advancing or shortening it) instead of using an implant. This technique takes a little longer and the incision is larger, although it is also made inside the mouth.
Types of mentoplasty
Augmentation mentoplasty
Indicated when the chin is under-projected or receding. It can be performed by placing an implant or by advancing the bone itself to improve the facial profile and balance the face.
Reduction mentoplasty
Recommended when the chin is too prominent or bulky. It involves reducing and reshaping the bone to achieve a softer-looking face.
Reshaping mentoplasty or osteotomy
Used to reposition the chin bone itself (moving it forward, moving it back or centering it) and correct asymmetries or disproportions, achieving a more precise and natural result.
What does chin surgery at IdB involve?
Mentoplasty is a cosmetic surgery procedure that changes the shape, size or position of the chin to improve facial harmony and the balance of the profile. It can be performed to augment, reduce, advance, set back or redefine the chin according to each patient's anatomical needs.
There are two main techniques: a chin implant and reshaping of the bone itself (osteotomy). The implant is the simplest option when only projection is lacking; an osteotomy allows the chin to be moved forward or back, shortened or centered. The technique and the type of anesthesia, local or general, are decided at your assessment according to each patient's anatomy.
Personalized medical assessment
Your consultation will always be with the surgeon who is going to perform your procedure. In this visit we analyze your facial structure, chin projection, profile, facial proportions and your aesthetic goals.
A payment of €80 is required in advance to book the appointment. If you cannot travel, the first visit can also be done by video call with the surgeon who would operate on you: it costs €50, which is deducted if you decide to have surgery.

Mentoplasty with Synapta®: a more manageable recovery
Traditionally, mentoplasty has been associated with a recovery involving swelling, a feeling of pressure and some discomfort in the chin and jaw area. At Instituto de Benito we have evolved this experience with Synapta®, an approach developed by specialists to support the body during recovery.
How is recovery after mentoplasty improved?
Unlike approaches focused solely on medication, Synapta® works on the body's response after the procedure. Using specific techniques applied at key points, the aim is to make the post-operative process more manageable and gradual, especially in an area as visible and structural as the chin.
- Relief of tension in the treated area: We work on the chin area and the perioral muscles to reduce the feeling of pressure or tightness, supporting a more comfortable recovery.
- Improved tissue condition: Oxygenation and drainage are encouraged, helping to reduce swelling and optimize the natural recovery process.

Who is not a candidate: contraindications for mentoplasty
Although mentoplasty is a facial surgery with a very well-established safety profile, there are situations in which it is contraindicated or should be postponed. In the pre-operative assessment we always analyze the patient's general health, facial anatomy, emotional stability and expectations before recommending any surgical procedure. An appropriate mentoplasty does not depend solely on the desire to change the chin, but on there being safe medical and anatomical conditions to perform it and a realistic expectation of the result.

| Untreated dental problems or periodontal infections | In mentoplasties with an intraoral approach, any source of dental infection increases the risk of surgical contamination and deep infection. |
| Body dysmorphic disorder or unrealistic expectations | When there is a distorted perception of one's facial image or expectations that are impossible to achieve surgically, surgery will not resolve the patient's dissatisfaction. A prior psychological assessment may be required. |
| Under 18 years of age | Facial bone development must be complete before considering a mentoplasty for cosmetic purposes, except for specific reconstructive indications. |
| Active autoimmune diseases or significant immunosuppression | They can increase the risk of poor healing, infection and complications related to implants or bone fixation. An individualized assessment is required. |
| Treatment with anticoagulants or antiplatelet drugs | This is not an absolute contraindication, but it requires adjustment or temporary suspension supervised by the prescribing physician and coordinated with the surgical team. |
| Recent facial surgery or injuries that have not stabilized | It is necessary to wait for the tissues and bone structures to recover properly before performing a new cosmetic procedure. |
| Pregnancy or breastfeeding | Although the surgery does not directly affect pregnancy, any elective cosmetic procedure should be postponed until this stage is over, for anesthetic safety and proper recovery. |

Risks and possible side effects of mentoplasty
Mentoplasty is a facial surgery with a very well-established safety profile when performed by qualified specialists, in an accredited surgical setting and after a proper anatomical and functional assessment of the patient. Whether performed with an implant or through bone advancement, it is a highly protocolized procedure whose aim is not only to improve the facial profile, but also to maintain the harmony and function of the jaw region.
At Instituto de Benito we work under strict medical criteria, with individualized facial planning, an accredited operating room and full anesthetic monitoring by a qualified anesthesiologist. Surgical safety, a natural-looking result and preservation of function are priorities in every case.
If after surgery you experience severe pain that does not improve with the prescribed medication, a fever above 38 °C, significant progressive swelling, difficulty breathing, persistent bleeding, discharge from the wound, significant changes to your bite or sudden loss of sensation, you should contact the clinic immediately or go to the nearest emergency department. Most complications can be properly resolved when detected early.
Expected, mild reactions
(usually self-limiting during the first 7–10 days)
- Moderate swelling of the chin and jaw.
- A feeling of facial tightness or pressure.
- Discomfort when speaking, smiling or chewing.
- Mild bruising in the treated area.
- Temporary reduction in sensation in the chin or lower lip.
- A temporary feeling of facial stiffness.
- Minor discomfort inside the mouth if the approach is intraoral.
- Slightly more pronounced swelling in the mornings during the first few days.
Less common effects
- Mild temporary asymmetry during the initial swelling.
- Hypersensitivity or temporary altered sensation in the lower lip.
- More visible scarring than expected with external approaches.
- Swelling that lasts beyond the first few weeks.
- A feeling of movement or gradual adaptation to the implant during early recovery.
- Discomfort when sleeping face down or resting on the jaw.
- Fibrosis or partial hardening of the soft tissues.
- Mild irregularities that can be felt during the healing phase.
Uncommon complications
(greatly reduced when the surgery is performed with the correct indication and proper technique)
- Surgical infection.
- Significant postoperative hematoma.
- Displacement or rotation of the chin implant.
- Localized bone resorption caused by pressure from the implant.
- Persistent sensory injury to the mental nerve.
- Residual facial asymmetries.
- Poor healing or wound dehiscence.
- Implant extrusion (very rare).
- Need for secondary revision surgery.
- Functional bite changes in cases associated with previously undiagnosed jaw imbalances.
- General anesthetic complications, which are exceptional after a proper preoperative assessment.
Minimizing the risks of a mentoplasty depends on the surgical technique, but also on a correct medical indication, precise facial planning and close post-operative follow-up. In each case we assess the chin, the balance of the face and jaw function before recommending the procedure.

Frequently asked questions about mentoplasty
What results can I expect?
A more balanced, harmonious and proportionate facial profile, with a chin suited to the proportions of your face. Full recovery comes between 6 weeks and 3 months, once the tissues have fully settled.
Is mentoplasty painful?
Discomfort is usually mild to moderate and is effectively controlled with medication and advanced recovery protocols. Most patients return to their usual routine in 7-10 days.
Will there be visible scars?
In most cases, no. The incision is usually made inside the mouth, leaving no visible scar. In some cases a small incision is made under the chin, where it is concealed in the natural crease. When a mentoplasty is combined with a neck lift, that same incision is used for both procedures.
Is the result permanent?
The result is stable and long-lasting, both with implant techniques and with bone reshaping techniques. As with any surgery, a touch-up may be needed in rare cases.
Can you see or feel a chin implant?
Today's implants are designed to integrate with the tissues and to look and feel natural. In rare cases the implant may shift or be felt to the touch, which is assessed at your follow-up visits.
When can I exercise or go in the sun?
Exercise: gradually from 3–4 weeks onward. Sun: after 4–6 weeks, always with high sun protection on the area.
Can mentoplasty be combined with other surgeries?
Yes. It is often combined with rhinoplasty, double chin liposuction or other facial procedures, always after a personalized medical consultation.
What happens if I gain or lose weight afterward?
Moderate weight changes do not usually affect the correction, which is made on the bone structure or with an implant. Significant changes can alter the contour of the chin and neck, so it is advisable to have surgery at a stable weight.
How long does full recovery take?
Most patients return to normal life in 7 to 10 days, although full recovery comes between 6 weeks and 3 months.
