What does this surgery involve?

It helps to understand the price, because a fat transfer is two surgeries in one. First, fat is removed with liposuction from areas such as the abdomen, flanks, back or thighs. That fat is centrifuged and purified to keep the cells that are useful. Then it is injected into the buttocks to add volume and shape, without implants.

The operation takes between two and four hours, under general anaesthesia, and the patient stays one night in hospital.

What does the price depend on?

On what each body needs. We work with three types of buttock fat transfer, basic, high-definition and structural, and they do not take the same time or the same work. The €7,500 is the starting price of the basic one. High-definition and structural fat transfer take more work and cost more.

Volume matters too. The more fat that has to be removed, and from more areas, the longer the operating time. And there is a limit that has nothing to do with money: there has to be enough fat to transfer. In very slim patients the technique is not viable, and that becomes clear at the first visit.

Safety and price go together

When comparing BBL quotes (Brazilian butt lift, as this surgery is known in English), it is worth asking how the fat is injected. We place it in the subcutaneous or subfascial plane, never inside the muscle, and we operate in a hospital operating theatre under general anaesthesia with monitoring. This is how the risk of fat embolism, an exceptional but serious complication, is reduced. Working this way has a cost, and it is included in the price.

What does the price include and what is paid separately?

What is included

  • Specialist's fees
  • Fees of the rest of the medical team
  • Materials specified in the quote
  • Operating theatre and one night in hospital
  • Anaesthesia
  • Private room if needed
  • Medication during the hospital stay
  • Nursing wound care
  • Follow-up visits with the specialist
  • Pre-operative tests (blood tests, ECG, X-ray, etc.)

What is not included

  • Assessments or clearances from other specialists, if needed
  • Medication after hospital discharge
  • Post-operative garments (compression garments, bands, cushion, etc.), unless specified
  • Complementary recovery treatments (lymphatic drainage, physiotherapy, laser, etc.)
  • Costs arising from medical complications not attributable to the surgery
  • Re-operations or touch-up surgery not planned initially
  • Additional hospital stays not planned
  • Travel, accommodation or meals for the patient and companions
  • Sick leave, certificates or additional paperwork
  • Any material or procedure not expressly specified in the quote

There is one point specific to this surgery. Part of the fat is naturally reabsorbed during the first weeks, and the fat that survives stays stable. If an additional grafting session were ever needed because of that reabsorption, it is not included in the initial quote.

The first visit and financing at Instituto de Benito

The first visit costs €80, which is deducted from the surgery if you decide to go ahead. It can also be done by video call with the surgeon who would operate on you, for €50, which is also deducted.

Buttock fat transfer can be financed over up to 12 months interest-free, or over longer terms of up to 48 months at a fixed interest rate. Amounts from €1,000 to €21,000 can be financed.

In Barcelona I operate, and the surgery includes the Synapta® protocol, designed to make the recovery easier. In Madrid it is performed by Dr. Antonio Martínez Murillo.

Frequently asked questions about buttock augmentation cost

What happens if there are complications? Is it included in the price?

The quote includes nursing wound care and follow-up visits, so post-operative follow-up is included in the price. It does not include the costs of medical complications not caused by the surgery, re-operations or touch-ups that were not planned, or hospital stays that were not planned. If any problem appears, it is assessed by the same surgeon who operated on you.

Is it covered by the Spanish public health system?

No. The Spanish public health system does not cover buttock augmentation, because it is cosmetic surgery. You can pay in instalments, over up to 12 months interest-free or up to 48 months at a fixed interest rate. The terms are on the financing page.

What is the best technique for buttock augmentation?

The one that suits your body. We work with three types of fat transfer: basic, high-definition and structural, which works on the deep and superficial subcutaneous planes to improve shape, projection and firmness when a more complete reshaping is needed. The choice depends on how much fat can be harvested, the quality of the tissues and the result you are looking for.

Is buttock augmentation safe?

Like any surgery it has risks, and most of the discomfort is part of a normal recovery. The most serious complication is fat embolism, which is exceptional. To reduce that risk we inject the fat in the subcutaneous or subfascial plane, never inside the muscle, and we operate in an accredited operating theatre with full anaesthetic monitoring. At the first visit we also check for any condition, such as a high risk of thrombosis, that would make surgery inadvisable.

Does it last forever?

The fat that integrates into the buttock stays, as long as your weight stays stable. Moderate changes do not usually affect the result. Major weight changes can change the volume. The result starts to show within a few weeks and full recovery comes between three and six months, once the fat has integrated.

How long is the recovery?

Normal life and desk work resume after ten to fifteen days. Light exercise returns at three or four weeks, and intense exercise from six or eight weeks, so that the grafted fat gets a good blood supply. Full recovery comes between three and six months.

What is the post-operative period like: compression garments and posture?

You need to wear compression garments on the areas where the fat was taken from. During the first weeks you must not sit directly on your buttocks and you have to sleep face down or on your side, always following the surgeon's instructions. Compression garments and the post-operative cushion are not included in the quote.

Does it leave scars?

Very small ones. They are the incisions through which the thin cannulas go in to remove the fat and inject it into the buttock. In patients prone to hypertrophic scars or keloids they may heal less well, and that is assessed before surgery.

What pre-operative tests are needed?

A blood test, an electrocardiogram and an X-ray, as well as your medical history and the examination at the first visit. They are included in the price.

Do implants come with a warranty?

No implant is used in a fat transfer. The volume comes from your own fat, so there is no manufacturer's warranty: the fat that integrates into the buttock stays, as long as your weight stays stable. Buttock implants do come with the manufacturer's warranty, under its own terms.

Implants or your own fat?

They are two different routes with different prices. In the article on implants or your own fat we explain when each one is the better option.
